With the assistance of a lawyer, draft a Statutory Declaration in verification of either your name, age, or date of birth.
The Statutory Declaration shall be commissioned by the commissioner of oaths.
Present the same to the official or I institution in need of the same.
Note. This procedure changes when one is desirous of changing or abandoning the use of his/her former names for the new name. In that regard, you will need to follow the following procedure:

The Deed Poll should be commissioned by the commissioner of oaths.
Upon pay6 of the prescribed fee, the Deed Poll must be gazetted by the Uganda Gazette.
You will receive a Gazette notice implying that the general public is on notice regarding the new name name adopted.
